Understanding CMS Latest Five Star Quality Rating Methodology

Resident w/Care giver

With the recent July 2024 refresh to Care Compare, CMS has changed the staffing case mix adjustment to a model based on the PDPM, essentially using the nursing case mix groups to adjust for acuity. Other important changes include:

  • The gap period used in defining termed staff has increased from 60 days to 90 days in the staffing turnover measures. This is good news for facilities who have staff that qualify for extended leave periods.
  • The risk adjust model for the claims-based measures has been adjusted to accommodate the shift from Section G to Section GG.
  • As anticipated, the thresholds for the staffing measures and the claims-based quality measures have been adjusted to maintain the same distribution by points for affected staffing and quality measures.
